Please go back to the home page and click "Schedule a Meeting: New Client Inquiry." After you set an appointment, you'll receive emails to sign in to my secure portal so you can sign your consent form. Also, please enter your address, birth date, Medicare number, Part A and B start dates, and prescriptions. If you are interested in a Medicare Advantage plan, also enter your Doctors. If you are only interested in a Medicare Supplement (Medigap) plan, then we don't need Doctors.
I will need the Medicare consent form digitally signed and your prescriptions and Doctors (for Medicare Advantage) before we can discuss details.

Please sign the ACA Consent form here and you can also use the "Schedule a Meeting: New Client Inquiry", but ignore the consent form for Medicare that will be automatically requested.
